 If you need a blue card for your work in one of the categories of employment or business regulated by the Commission’s Act, you need to complete the appropriate application form, and forward it to the Commission.

Before completing the relevant application form please read:

How to apply for a blue card which aims to help you fill out the form.

Application to cancel a negative notice

Authorisation to confirm a valid blue card form

An authorisation to confirm a valid blue card form can be used by an applicant or cardholder, to advise the Commission that they consent to the Commission providing the following information to an authorised person:

  • the current status of a blue card application
  • the validity of a blue card
  • any changes to the status of a blue card application

Upon lodgement of a completed authorisation, the Commission will confirm the current status in writing to the authorised person.


Child care services

The Commission is responsible for screening people who operate or are engaged in licensed child care services i.e. home based and centre based facilities. It also screens stand-alone child care services and commercial child care services such as babysitting, nanny services and other adjunct care services.

Child care paid employees

A person who is employed in a paid capacity:

  • as a carer in, or staff member of a child care service
  • to provide child care in the course of a commercial service other than a child care service - i.e. babysitting, nanny, short term care in a hotel or resort, adjunct care

is taken to be carrying out paid employment and must submit a CCP form.

Child care volunteers

A person who is employed in a voluntary capacity:

  • as a carer in, or staff member of a child care service
  • to provide child care in the course of a commercial service other than a child care service - i.e. babysitting, nanny, short term care in a hotel or resort, adjunct care

is taken to be carrying out voluntary employment and must submit a CCV form.

Persons carrying on childcare businesses

A person who carries on a child care business as a:

  • Licensee / Director or Nominee of a centre based service
  • Licensee / Director or Nominee of a home based service
  • Licensee / Executive Officer of a school age care service
  • Certain positions within a Commercial Child Care Service

is taken to be carrying on a regulated business and must submit a CCB form.

Family Day Care

Carer

A Carer in a Licensed home based service or In-Home Carer providing a child care service through an arrangement with a Licensed Scheme may apply for a blue card by submitting the Family Day Care, FDC - Carer application form. This form must be accompanied by the $40 application fee. The Carer's identification must be sighted by the Family Day Care Scheme. An information sheet is provided below to guide carers and schemes in completing this application form.

Adult Occupant

A Carer in a Licensed home based service must not provide child care in the carer's home unless each adult occupant of the home has a current blue card. An adult occupant is any person who is 18 years or over who resides in the home, or is usually present in the home when child care is being provided. If you are unsure whether you are an adult occupant and require a blue card you should contact the Family Day Care Scheme. The Scheme will seek clarification from the Office for Children at the Department of Communities.

An adult occupant may apply for a blue card by submitting a Family Day Care FDC - Occupant application form. The applicant's identity must be verified by a Justice of the Peace, Lawyer, Police Officer or Commissioner for Declarations. This application is processed free of charge.

Child Protection Services

The following people associated with Child Protection Services need a blue card:

  • foster carers and kinship carers
  • adult members of a foster carer’s or kinship carer’s household
  • paid employees of a Licensed Care Service
  • nominees and directors of Licensed Care Services
  • shared family care providers
  • people carrying out work inside a licensed residential facility (ie ancilliary service providers, such as maintenance workers, electricians, technicians)

Blue card application forms for paid employees of a Licensed Care Service and nominees and directors of Licensed Care Services can be accessed from the Department of Child Safety’s website at www.childsafety.qld.gov.au.

People carrying out work inside a Licensed Residential Facility in their capacity as a paid employee of an ancillary service provider should complete a Paid Employee (P) Application Form . If the person is a self-employed ancillary service provider, they should complete a Business (B) Application Form .

Volunteers of a Licensed Care Service or an ancillary service provider at a Licensed Residential Facility should complete a Volunteer (V) Application Form .
